4P Program | Managing High Blood Pressure In Patients | Middlesex Hospital



The Middlesex Hospital Family Residency Program, led by 3rd year resident Kristin Gates, MD, collaborated with the Middletown Community Health Center and Chatham Health District in a highly successful trial of nurse care managers in improving the control of high blood pressure in patients through home based self-monitoring.

The Middlesex 4P self-blood pressure monitoring pilot is a collaborative effort in engaging Patients and Providers in the Partnership to Prevent Heart Disease.
